The Covington Health Department is offering back to school screenings and immunizations for a discounted price.

All screenings done through the health center will be documented on the required Georgia Form 3300. The hearing and vision screening is $15 and the dental screening is $10, for a total of $40. With proper documentation, records from previous hearing, vision and dental screenings conducted within the last 12-months can be transferred and placed on the needed from for a $4 fee.

All immunizations done at the health department will be documented on the required Certificate of Immunizations Form 3231. If a child does not meet the Georgia requirements, the department can vaccinate with the needed immunizations. Fees start at $14.75 per shot. With proper documentation, all out-of-state immunization records can be transferred onto the required forms for an additional $4.
