Newton County School System will hold centralized registration for the 2009-2010 school year beginning July 1 , Monday through Thursday, from 9 a.m. until 4 p.m. at Clements Theme School, located at 66 Jack Neely Road. Signs will be posted. Documents needed for student enrollment are as follows:
o Two proofs of residency – a utility bill and lease agreement or mortgage statement
o Parent/guardian photo ID
o Proof of custody/guardianship if applicable
o Copy of birth certificate
o Copy of social security card or waiver signed
o Immunization record – GA Form 3231
o Clinic information card
o Georgia certificate of eye, ear and dental examination – this is only needed for students entering a Georgia public school for the first time or re-entering a Georgia public school after being gone for one entire school year.
o Copy of most recent report card
o Copy of most recent withdrawal form
o Test score result form
o Special Education records if applicable
o Copy of most recent Discipline Report – 7th through 12th grade only
